how do I tell when it is time to add another box?

I got into my hive and the top box it looks like all 10 frames are all combed and half full of honey... how do I know when it is time to add another super on the top?

There are two quick and easy rules of thumb:

1) 7 of the 10 frames are filled with either brood or nectar/honey.
2) There is white wax on the tops of the frames, indicating that the nurse bees are making wax and need to do SOMETHING with it.
